The three most common specimen types are oral fluid (saliva) — easy to collect, observable, with low risk of substitution; capillary blood (finger-prick) — minimally intrusive with high sample integrity; and urine — longer detection window, but unobserved collection raises tampering risk without strong controls.

Most providers use immunoassay screening, which is fast and cheap but prone to false positives and false negatives. Confirmation should be done with LC-MS, the gold standard. Detection windows vary by substance, dose, frequency, metabolism, and specimen type. Oral fluid and blood reflect more recent use; urine can detect some substances for several days; hair can show exposure over weeks to months, though it's uncommon in workplace programs due to privacy and interpretation complexity.

In most workplace settings, testing is based on informed consent and a clear drug & alcohol policy, referenced in employment terms or onboarding. A structured, defensible process: review of the laboratory result, confirmation where applicable (LC-MS/MS is the gold standard), Medical Review Officer review to assess legitimate medical explanations, and employer action per internal policy — often including support measures, return-to-work planning, and follow-up testing.

LC-MS (liquid chromatography–mass spectrometry) is the highest-precision method available for drug analysis. It’s the same technology used by police forensic labs - capable of identifying substances at trace levels with near-zero false positives. Unlike immunoassay screening, which is common but imprecise, LC-MS produces results that hold up legally and operationally.
